A Shift in Our Understanding of Fibromyalgia: Why New Research Is Changing the Conversation

For years, fibromyalgia has been one of the most misunderstood chronic pain conditions. Many people living with fibromyalgia have been told that their symptoms are “just stress,” that their tests are normal, or that there isn’t much that can be done beyond managing symptoms.

Fortunately, research over the past decade has dramatically changed how healthcare providers understand fibromyalgia—and with that shift comes new opportunities for more effective treatment.

Fibromyalgia Is More Than Widespread Pain

Fibromyalgia is now widely recognized as a condition involving centrally mediated pain. This means the pain isn’t simply coming from injured muscles or joints. Instead, the central nervous system—the brain and spinal cord—becomes sensitized.

When this happens, the nervous system develops a lower threshold for detecting pain. Sensations that may not normally be painful can become uncomfortable, and painful experiences may feel significantly more intense. This process is known as central sensitization.

Understanding this distinction is incredibly important because treating centrally mediated pain requires a different approach than treating an acute injury like a sprained ankle or pulled muscle.

Part of a Larger Group of Chronic Pain Conditions

Fibromyalgia is considered one of several Central Sensitivity Syndromes and overlaps with a group of conditions known as Chronic Overlapping Pain Conditions (COPCs).

These conditions frequently occur together and may share similar nervous system changes, including central sensitization. Examples include chronic pelvic pain, irritable bowel syndrome (IBS), temporomandibular disorders (TMJ), migraine, and interstitial cystitis/bladder pain syndrome.

Rather than viewing each diagnosis in isolation, clinicians are increasingly recognizing that many of these conditions may share common neurological mechanisms.

What the Brain Tells Us

Advances in brain imaging continue to deepen our understanding of fibromyalgia.

One important area is the precentral gyrus, which sits adjacent to the brain’s sensory mapping regions. The nearby sensory homunculus represents how the brain processes sensation from different parts of the body. Researchers believe that altered sensory processing may contribute to the widespread pain experienced by people with fibromyalgia.

Another area receiving significant attention is the insular cortex. This region helps regulate both visceral sensations—signals coming from internal organs—and the autonomic nervous system, which controls functions like heart rate, blood pressure, digestion, and temperature regulation.

Research has demonstrated altered connectivity within the insular cortex in people with fibromyalgia, suggesting that changes in how the brain communicates may contribute to both pain and many of the non-pain symptoms patients experience, including fatigue, dizziness, digestive issues, and sensory sensitivity.

New Research Highlights Comprehensive Treatment

A recent review examining both pharmacological and non-pharmacological treatments reinforces what many clinicians specializing in chronic pain have observed: there is no single solution, but a comprehensive approach can be highly effective.

Among medications, Low-Dose Naltrexone (LDN) continues to gain attention for its potential to reduce centrally mediated pain. While additional large-scale studies are still needed, current research suggests LDN may help decrease nervous system sensitivity for some individuals living with fibromyalgia.

Equally important are non-pharmacological interventions, including:

  • Physical therapy focused on nervous system regulation rather than simply strengthening
  • Graded movement and exercise programs
  • Pain neuroscience education
  • Sleep optimization
  • Stress management strategies
  • Cognitive and behavioral approaches
  • Autonomic nervous system regulation

The strongest evidence continues to support combining multiple treatment approaches that address the nervous system as a whole.

Why This Matters

Over 15 years ago, when I began working as a physical therapist, I remember feeling concerned whenever I met someone carrying a diagnosis of fibromyalgia.

Too often, it seemed to function as an umbrella diagnosis—one that explained a patient’s symptoms without truly helping them understand why they were experiencing pain or what treatment options existed. Many patients left appointments believing nothing could be done.

Today, our understanding is much more sophisticated.

Fibromyalgia is not simply persistent pain, nor is it the same as pain from a recent injury. It reflects real, measurable changes in how the nervous system processes sensory information. Recognizing these changes allows healthcare providers to develop more targeted, evidence-based treatment plans while validating the lived experiences of those with the condition.

As research continues to evolve, one message is becoming increasingly clear: fibromyalgia is a complex neurological pain condition, and with the right combination of education, rehabilitation, medication when appropriate, and individualized care, meaningful improvement is possible.
