What to Know Before Your Brooklyn Appointment

Our office is located in a ground floor/street level storefront at 275 4th Avenue Brooklyn, NY 11215.

Cancellation Policy:

If you do not cancel within 48 hours, you forfeit the full payment of your visit.

Policies and Important Information:

  1. Inspira is a cash-based Physical Therapy practice that is out of network for all major medical insurances. Patients are responsible for the full cost of the appointment upfront and can submit for out-of-network reimbursement.
  2. We do not accept Medicare at our Manhattan and Brooklyn offices.
  3. If you wish to have Inspira submit PT insurance claims as a courtesy on your behalf, you  MUST email billing@inspirapt.com to make this request and include a photo of the front and back of your insurance card and reconfirm DOB.
  4. Out-of-Network Policy. Upon request, we will reach out to your health plan to determine whether you have out-of-network benefits for physical therapy services and what those benefits cover. If you have out-of-network benefits, we can submit claims to your health plan as a courtesy to you if you request. But you will remain responsible for paying us in full for the services that we provide to you. You are responsible for obtaining any necessary physician referrals and/or pre-authorizations for services. We are not responsible if your health plan denies, in whole or in part, the claims for our services. Please note, although the insurance company may tell us a percentage of what they will reimburse, it is not a guarantee of reimbursement. Many times the reimbursement reflects a percentage of a usual and customary rate that your insurance company determines.
  5. We will communicate with patients’ referring provider, collaborating provider, and insurance company for insurance approval. However, our communication & collaboration does NOT guarantee in any way approval by insurance for physical therapy care and/or other procedures. Please keep this in mind if the out of pocket upfront cost is a concern for you.
  6. Our preferred providers operate under different entities and Inspira Physical Therapy is not the same business as our preferred providers.  Therefore, our business practices such as billing procedures will differ.  We do NOT communicate billing information or gather billing information from other preferred providers. Please Refer to #3 for more details on courtesy claim submission.
  7. Major medical insurance companies often deny reimbursement for skilled Physical Therapy care for various reasons or no valid reason besides an internal corporate quota they must meet.
  8. In 2024 we found some insurance companies will reimburse more if the patient submits the claim for their Out of Network care versus if the out of network provider (Inspira PT) submits the claim for the patient (you).  This can sometimes be discovered by a phone call, other times the insurance is not forthcoming with this information.  **This is new and we have never encountered this before 2024.  If you discover this for your plan please let us know and we will email the claims to you for you to submit on your behalf.
  9. Insurance companies can change their policies and procedures with little to no notice mid year.  Example, they may begin requiring pre-authorization in April when they did not in January of the same year.  Please keep in mind the fluid nature of health insurance plans policies.  We will do our best to communicate what we know.
  10. Home Exercise Progams (HEP’s):  if you do not receive your home exercise program please contact the front desk at admin@inspirapt.com or call/text 929-295-6566.
  11. Referrals: A referral is needed in order to start PT, or for women in their 1st or 3rd trimester. For all other patients, according to the NY State Direct Access Law, after 10 sessions or 30 days, a prescription is required. Please let us know who your referring provider is so that we can reach out to them for your prescription. Or, referring providers can fax us the referral at 929-220-5721.
