At Brooklyn Inspira Physical Therapy, our pelvic floor therapists work with people of all ages and genders to restore function, reduce pain, and improve quality of life through personalized treatment plans. Our goal at Inspira Physical Therapy is to improve your quality of life by strengthening the pelvic floor muscles.
Your pelvic floor consists of 26 interconnected muscles, ligaments, and connective tissues that form a hammock-like structure at the base of your pelvis. These muscles serve four essential functions in your daily life.
When these muscles become too weak (hypotonic) or too tight (hypertonic), you may experience various symptoms that affect your comfort and daily activities.
The Pelvic Floor muscles are located between the tailbone (coccyx) and the pubic bone within the pelvis.
They support the bowel and bladder and are important in sexual function. They can become weak over time due to factors such as heavy lifting, being overweight, constipation, childbirth, menopause, and surgery.
This may lead to symptoms including vulvar pain, localized pelvic discomfort, prolapse, low back pain, leakage, incontinence, dysfunction, and much more.
These symptoms often connect to specific diagnoses, including hypertonic pelvic floor, vulvodynia, vestibulodynia, and lichen sclerosus.
Many pelvic health conditions have other overlapping health conditions such as Hypermobility, Ehlers Danlos syndrome and different autoimmune disorders.
Understanding which type affects you is crucial for developing an effective treatment plan.
Hypertonic pelvic floor occurs when muscles are chronically tight and unable to relax properly. This tension can cause pelvic pain, painful intercourse, constipation, and difficulty emptying your bladder completely. Common causes include chronic stress, trauma, poor posture, or compensatory patterns from other injuries.
Hypotonic pelvic floor happens when muscles are weak or overstretched, making it difficult to contract them effectively. This can lead to urinary incontinence, pelvic organ prolapse, or a feeling of heaviness in your pelvis. Pregnancy, childbirth, aging, chronic coughing, and repetitive heavy lifting can contribute to muscle weakness.
Pelvic floor physical therapy effectively treats numerous conditions affecting your pelvic health.
For urinary issues, we address stress incontinence (leaking with coughing or sneezing), urge incontinence (sudden, strong urges), overactive bladder, and painful urination.
Bowel-related conditions include constipation, fecal incontinence, and irritable bowel syndrome symptoms.
We also treat various types of pelvic pain including chronic pelvic pain, painful intercourse (dyspareunia), vulvodynia, vaginismus, endometriosis-related pain, and tailbone pain.
Our prenatal and postnatal pelvic floor physical therapy helps with pregnancy preparation, postpartum recovery, diastasis recti (abdominal separation), and cesarean scar tissue.
Additionally, we work with pelvic organ prolapse, erectile dysfunction in men, prostate pain, and post-surgical rehabilitation including gender-affirming procedures.
We provide specialized pelvic floor therapy for diverse needs including men’s health, postpartum recovery, post-surgical rehabilitation, and chronic pain management, delivering personalized treatment plans that restore function, eliminate discomfort, and improve quality of life.
Your first pelvic floor physical therapy appointment begins with a comprehensive discussion about your medical history, symptoms, and goals.
During the initial 50-minute evaluation, your pelvic health specialist will perform an external assessment of your posture, core strength, hip mobility, and movement patterns. If appropriate and with your consent, an internal pelvic floor exam may be performed using one gloved finger to assess muscle strength, coordination, and areas of tension or tenderness. This exam differs from a gynecological exam—we don’t use stirrups or a speculum, and you remain in control throughout.
Treatment techniques may include manual therapy (gentle internal or external massage), biofeedback to help you visualize muscle activity, therapeutic exercises, electrical stimulation for muscle strengthening, and breathing techniques to promote relaxation.
Most patients attend one to two sessions weekly for 8-12 weeks, though this varies based on individual needs.
Pelvic floor therapy benefits people of all ages, genders, and life stages.
Women’s health physical therapy commonly addresses pregnancy and postpartum concerns, menopause symptoms, and gynecological conditions.
Men benefit from treatment for post-prostatectomy incontinence, erectile dysfunction, and chronic prostate pain. Male pelvic floor physical therapy is increasingly recognized as essential care for prostate health, bowel and bladder control, and sexual function.
If you’re experiencing any changes in bladder or bowel control, pelvic discomfort, or sexual dysfunction, consulting a pelvic floor specialist can help determine if therapy is right for you.

We provide a private, comfortable space for discussing sensitive concerns without embarrassment while collaborating with your healthcare team to ensure coordinated care.
Our therapists complete specialized fellowship training and board certification in pelvic rehabilitation, going far beyond basic physical therapy education. We're always looking for PTs who share that commitment. See current openings at Inspira.
Each pelvic floor treatment plan is individualized—we never use a one-size-fits-all approach. You'll work with the same pelvic health therapist throughout your care, building trust and rapport.
Your first visit includes a detailed discussion of your symptoms and a physical assessment. With your consent, this may include an internal exam to evaluate muscle function. Subsequent sessions involve hands-on treatment and prescribed exercises.
Most patients see improvement within 6-8 weeks, though some conditions require longer treatment. You'll typically attend one to two sessions weekly for 8-12 weeks.
Treatment should not be painful. You may experience mild discomfort during assessment or treatment of tight muscles, but you control the process and can ask your therapist to adjust at any time.
In New York State, you can directly access physical therapy without a physician referral. However, some insurance plans may require one for coverage.
No. While internal assessment is considered the gold standard for evaluating pelvic floor function, it's never mandatory. Your therapist can create an effective treatment plan using external techniques if you prefer not to have an internal exam.
Regular physical therapy treats muscles and joints you can see and reach from the outside. Pelvic floor therapy treats a group of 26 muscles inside your pelvis that control your bladder, bowel, and sexual function. It requires separate post-graduate training and board certification in pelvic rehabilitation, which not every physical therapist has.
Inspira is out-of-network with all insurers. We provide a superbill after each visit that you can submit to your plan for reimbursement, and many patients with out-of-network benefits get a portion back. We can explain what to expect before you book.
Common signs include leaking when you cough, sneeze, or run, pain during sex, a feeling of heaviness in your pelvis, constipation you cannot resolve, or pelvic pain that has lasted more than a few weeks. If any of these sound familiar, an evaluation will tell you whether the pelvic floor is involved.
